Terms and Conditions

Please read these Terms and Conditions carefully. All services provided by ReVoice Clinic shall be governed by them.  ReVoice Clinic requires clients to sign acceptance of Terms and Conditions before providing any services. 

Charges

The client shall pay agreed charges to secure an appointment. ReVoice Clinic will provide payment details to the client by email and require payment prior to the appointment by bank deposit.

Cancellation, changes to bookings and failure to attend appointments

For cancellations up to 24 hours before a scheduled appointment, ReVoice Clinic will reschedule the appointment at no additional cost, or in exceptional circumstances provide a full refund.

Cancellations must be received by email or text more than 24 hours before the scheduled appointment for the cancellation to be accepted.

For cancellations, changes to bookings or failure to attend an appointment with less than 24 hours’ notice, outside of exceptional circumstances: 

    1. The client will be liable to pay ReVoice Clinic the total fee.

    2. No refund given except at ReVoice Clinics sole discretion.

An exceptional circumstance is defined as an  event outside the control of the client, where its effect could not have been avoided. This may include unexpected technological issues related to telepractice. 

If the client fails to attend two or more appointments, ReVoice Clinic reserves the right to discharge the client from the service without notice.

If ReVoice Clinic needs to cancel an appointment for any reason, they may do so at any time before the appointment is scheduled to begin.  This event is not expected to occur unless there are exceptional circumstances. In this case the client will be offered a full refund for the appointment fees, or the client will be given a choice of alternative dates for the appointment, but ReVoice Clinic is not liable to compensate the client for any other expenses they might have incurred in connection with the appointment.  ReVoice Clinic will aim to notify the client of any cancellations well in advance, but they cannot guarantee to do so if it needs to be cancelled due to an exceptional circumstance at short notice. 

Attendance and Conduct

  1. The client is asked to ensure they arrive on time for each appointment. If they are late to their appointment, ReVoice Clinic has the right to limit the appointment length to finish at the scheduled time to respect their other clients. In this case, the client remains liable for the full appointment fee and they will not be issued a refund, unless this is due to a one-off exceptional circumstance. 

  2. In the event of Telepractice (online video therapy), the client must complete a ‘free consultation’ provided by ReVoice Clinic to ensure a stable connection and correct technology is available for effective therapy prior to paying their fees.  

  3. ReVoice Clinic reserves the right to refuse admission to the client or ask the client to leave the premises if ReVoice Clinic thinks the client is behaving in a disruptive way or in a way that violates a lease, or in a way that is likely to cause damage, nuisance, offence or injury.  In these circumstances the client will remain liable for the appointment fee and we will not issue any refund.

  4. By entering into this agreement the client agrees to ensure that they comply with all health and safety and other rules and regulations applicable to ReVoice Clinic’s premises.  The client must  also agree not to bring onto our premises any illegal or hazardous items. 

  5. No waiver: Any failure by ReVoice Clinic to enforce any provision of this agreement at any time shall not be construed as a waiver of such provision and shall not affect their right to enforce such provision at any time.

Telepractice (online therapy)

ReVoice Clinic will adhere to Aotearoa Allied Health Best Practice Guide for Telehealth 2020.

  1. ReVoice Clinic is responsible for informing the client about the limitations of telepractice and the client can make an informed choice, and the client is to give written consent before telehealth service is provided. 

  2. Revoice Clinic will consider whether a physical examination would add critical information before providing care. If a physical examination is likely to add critical information, they will not proceed until a physical examination can be arranged. In some circumstances, it may be reasonable to ask another practitioner in the client’s locality to conduct a physical examination on their behalf.

  3. ReVoice Clinic will conduct telepractice on a device, software or service that is secure and fit for purpose, and preserves the quality of the information being transmitted. 

  4. ReVoice Clinic will keep clear and accurate client records that report relevant clinical information; all options discussed; decisions made and the reasons for them; any information provided to the patient/client; and the proposed management plan. All correspondence with the client (via email, video or telephone) will be recorded in the client’s  notes indicating it was a telehealth intervention. 

Intellectual Property

  1. All materials provided to the client by ReVoice Clinic (our staff or associates) and any intellectual property belonging to or associated with our company and/or services: including, but not limited to, any website, trade mark, trade name, logo, audio-visual, text are the sole property of ReVoice Clinic (our staff or associates). The client will agree not to infringe any of ReVoice Clinic’s rights in any way.

  2. The client may make a copy of materials provided for their own personal use, but no other use of them is authorised.

Limitations and Exclusions of Liability

  1. Nothing in a contract under these Terms and Conditions will:

    1. limit or exclude any liability for death or personal injury resulting from negligence;

    2. limit any liabilities in any way that is not permitted under applicable law; or

    3. exclude any liabilities that may not be excluded under applicable law.

  2. ReVoice Clinic  shall not be liable to the client in respect of any special, indirect or consequential loss or damage.

  3. ReVoice Clinic reserve the right to refuse to treat the client if the presenting condition is not within scope of practice.

Confidentiality

  1. ReVoice Clinic will adhere to the Health Information Privacy Code 1994, and any sensitive information will be protected. They will fully inform the client about security issues specific to the use of email. 

  2. ReVoice Clinic are obliged by law to share information about the client if they are in serious danger in the immediate or foreseeable future, their competence to make a decision is impared, legal requirements demand that confidential material be revealed or they are responding to a complaint about their practice.   

Complaints

  1. ReVoice Clinic will adhere to the New Zealand Speech Language Therapy Association’s Code of Ethics at all times. The client should refer to the organisation's website regarding complaints policies should they have a concern or complaint about the service.
